Please read these Terms of Service (“Terms”) carefully. By scheduling an appointment, using our services, or visiting our clinic, you agree to be bound by these Terms.
1. Scope of Services
Insight Chiropractic Inc. provides chiropractic care, wellness consultations, and related health services. All services are provided by licensed professionals according to accepted standards of chiropractic care.
2. SMS Communications
2.1 Consent to Receive SMS Messages
By providing your mobile phone number and opting in, you consent to receive SMS text messages from Insight Chiropractic Inc.
These messages may include:
Appointment confirmations and reminders
Follow-up communications
Service-related updates
Billing notifications
Office announcements
Message frequency may vary.
2.2 Message & Data Rates
Message and data rates may
apply depending on your mobile carrier and service plan.
2.3 Opt-Out Instructions
You may opt out of receiving SMS messages at any time by replying:

HELP for assistance
You may also contact our office directly to request removal from SMS communications.
2.4 Carrier Disclaimer
Carriers are not liable for delayed or undelivered messages.
2.5 Age Restriction
SMS services are intended for individuals 18 years of age or older. If you are under 18, a parent or legal guardian must provide consent and be the primary contact for SMS communications.
3. Privacy Policy
Your privacy is important to us. Our collection and use of personal information, including mobile data, is governed by our Privacy Policy.

4. Patient Responsibilities
You agree to provide accurate, current, and complete health and personal information.
You understand that chiropractic care may involve hands-on procedures and agree to communicate openly with your provider regarding comfort or concerns.
You agree to follow the recommended care plan.
5. Appointments and Cancellations
We request at least 24 hours’ notice for cancellations or rescheduling.
Missed appointments or late cancellations may be subject to a $67 cancellation fee.
Repeated no-shows may result in discharge from the practice.
6. Payment and Insurance
Payment is due at the time of service unless other arrangements are made in advance.
We accept cash, credit cards, debit cards, and HSA. Insurance coverage remains your responsibility to verify.
7. Health Information and HIPAA
We comply with applicable federal and state privacy laws, including HIPAA.
Health records are confidential and shared only with written consent or as required by law.
8. Limitations of Care
Chiropractic care is not a substitute for emergency medical treatment.
If you experience a medical emergency, call 911 or go to the nearest emergency room.
We do not guarantee specific results.
9. Children and Minors
Patients under 18 must be accompanied by a parent or legal guardian.
10. Governing Law
These Terms are governed by the laws of the State of Florida.
